It's for anything international. Once you declare, that's the only option.

I'm assuming there are some exceptions to the rule. There always are. But I can't think of any off the top of my head.

I don't believe this is correct. The "4 year rule" allows for players to change the national eligibility once as long as they have played in said nation under a recognized hockey program for at least 4 years and have obtained the citizenship of that country

The Buffalo Sabres today announced that the team’s intra-squad scrimmage during development camp will be streamed live on Sabres.com. The scrimmage will take place on Tuesday, July 15 at 7 p.m. at First Niagara Center and is open to the public with free admission and parking.

The scrimmage will include three 20-minute, stopped-time periods. Sabres broadcast team Dan Dunleavy and Rob Ray will call the game, with Brian Duff conducting interviews during the intermissions.

This year marks the third year in a row that the ice has been installed during the summer and the development camp has been held at the home of the Sabres. Attendees take part in both on-ice and off-ice workouts. The goals of the camp are to show prospective players the fundamental ideas of the Sabres’ playing system and expose them to the conditioning programs of an NHL player. Camp participants also study and examine NHL rules and the successful techniques a player must possess to become a professional athlete.

Hard to apply those rules to players in the NHL , rules are written for amateurs it seems.
