They were trying to throw a piece of foam down where he was supposed to land, but instead he flew outside of the loop.

Even so, had they just dropped the foam where they were holding it, he would have at least had a somewhat cushioned fall. Instead, they chucked it through the thing. Also, assuming he made it through, wouldn't the foam have caused him to fall after he was done (inertia and all that jazz)?

Obviously a freak accident. What could've possibly gone wrong? They took all the safety precautions needed. Just a slight miscalculation on the foam toss.
