You are correct, the 10% bump is only for guys making $660K or less. 5% for $660K-$1M. And no raise for $1M+.
It doesn't really matter who asks for it, both the team and the player have an ask. It's just a housekeeping thing, I think if a team can only ask for arb once or twice.

A player can only be taken to arb once in his career, but can file for arb as many times as he wishes. There's a few differences between the two, but I doubt anyone cares.
If they don't see Lehner as their starter next year, they really don't want it to go to arb. If they qualify him, Robin would be a fool not to file. It's why I'd trade him now for whatever you can get.

It’s funny, because it could be fairly stated that this opinion of his qualifications to lead a hockey team is based on nothing but ... nothing.
But a 15 year vet of the league, 2 time captain in the league, 2 time Olympian (once as captain) and Stanley cup winner thinks quite differently. “That’s a guy who could easily be a captain.”

Having what it takes and the ability to identify someone who has what it takes are two completely different things. This is exactly why Wayne Gretzky was a colossal disaster as a head coach in the NHL.
